Warranty and Liability Information

Data Loss:I understand that data recovery and/or data backup may not be possible and a loss
of data and/or software may result from diagnostic testing and/or servicing. AV-PC-MediX assumes
no responsibility for any software program, data loss, data restoration, or data recovery. AV-PC-MediX
assumes no financial responsibility for data not recovered and/or unrecoverable.

Unclaimed Repairs: I grant AV-PC-MediX a security interest in and to the equipment to secure payment
of services incurred hereunder. I understand that any items not claimed and paid for within (30) days will
become property of AV-PC-MediX and all data will be removed from the hard drive. I understand that
AV-PC-MediX will make (5) attempts to contact me.

Repair Quote: Fees associated with our services, as well as additional fees for hardware parts and components,
will be quoted for your approval prior to the installation and/or performance of product or service. There is no
obligation to have any work performed. If we do successfully determine the cause of your PC's problem(s) and
you decline further service, a $60 diagnostic fee will be charged.

Defective Hardware Parts and Components: AV-PC-MediX does not warrant that all ordered and supplied
hardware parts and components will be free from defects in material and or workmanship as this is the
responsibility of the manufacturer. At the discression of AV-PC-MediX, AV-PC-MediX may assist in the
RMA of defective hardware parts and components for as long as the manufacturer’s warranty is valid.

In no event shall AV-PC-MediX be liable for any consequential or incidental damages whether if it be
software related or hardware related due to performance of services. The total liability of AV-PC-MediX
shall not exceed the total paid to AV-PC-MediX for services or products rendered.
